Definition
Very rare location for hemangioma.
Benign tumor.
About fifty cases reported in the literature.
Male-to-female ratio: 1/2.
Often unilateral
Clinical features
Asymptomatic.
May be symptomatic if very large due to mass effect
Laboratory findings
No abnormality on laboratory workup
Radiography
Noncontributory
Ultrasound
If visualized on ultrasound, heterogeneous adrenal mass syndrome
CT
Presence of calcifications (phleboliths) in two-thirds of cases.
More or less bulky nodular lesion.
Enhancement pattern consistent with an angioma, heterogeneous, peripheral centripetal nodular enhancement, complete on delayed phase.
MRI
Enhancement pattern identical to CT.
Usually heterogeneous lesion
Nuclear medicine
Not hypermetabolic on 18-FDG when characterizing a mass considered suspicious.
Management
Most cases reported in the literature were confirmed by pathology/surgical resection.
Therapeutic abstention if the diagnosis is certain.
Differential diagnosis
Adrenal metastases
Other primary adrenal lesion:
- pheochromocytoma
- cyst
- myelolipoma
- hematoma
- adrenocortical carcinoma
- ganglioneuroma/lymphoma
