Definition
Rare epithelial tumor of the ovary
Presence of transitional cells within a fibrous stroma
1 to 2% of ovarian tumors,
5% of benign epithelial tumors
Benign in 95% of cases
Age at diagnosis: 40-80 years (mean 50 years)
Unilateral in 93% of cases
Mean size < 2 cm
Associated with:
- benign mucinous tumor
- mucinous cystadenocarcinoma
- dermoid cyst
Clinical features
Incidental finding
CT
Solid tumor, central calcifications
MRI
Benign form:
Well-defined solid lesion,
T1 isointense,
marked T2 hypointensity,
diffusion hypointensity,
progressive enhancement (type 2 curve),
possible microcysts
Borderline or malignant form (exceptional):
Mixed with associated cystic component,
tissue portions with vegetations of intermediate T2 signal,
diffusion hyperintensity,
enhancement with type 2/3 curve
Differential diagnosis
Benign form:
Fibrothecoma,
subserosal pedunculated uterine fibroid,
Sertoli cell tumor,
Leydig cell tumor,
granulosa cell tumor
Malignant form:
Other epithelial tumors (serous and mucinous cystadenocarcinomas, clear cell cystadenocarcinoma, carcinosarcoma)
