Definition

A phenomenon of adipose tissue necrosis, causing local inflammation and pain.

Pericardial fat necrosis is a benign cause of chest pain, the incidence of which is probably underestimated.

Clinical features

Lateralized chest pain, which may worsen on inspiration

Normal ECG

No cough or dyspnea

CT

Oval, well-defined, hypodense mediastinal mass with peripheral enhancement

Possible small-volume pleural effusion.

Management

Symptomatic treatment with analgesics (step I) and NSAIDs

Rapidly favorable course
