Definition
A myocardial bridge is the segmental overlap of a coronary artery by a myocardial band along its normal epicardial course.
Clinical features
In the vast majority of cases: asymptomatic and an incidental finding, particularly on coronary CT angiography.
Rarely:
- Systolic compression leading to myocardial ischemic symptoms: resting angina, acute coronary syndrome, notably with anterior infarctions given the preferential location on the LAD
- Rhythm and conduction disorders
- Syncope
- Sudden death
CT
On coronary CT angiography, the following should be specified:
- the arterial segment involved,
- the length of the bridge: long myocardial bridge if >= 25 mm
- and the depth, i.e., the thickness of myocardium between the most superficial arterial wall and the epicardial fat.
Superficial myocardial bridge if < 2.5 mm. - Deep myocardial bridge if >= 2.5 mm.
