Definition

Osteonecrosis that may be of ischemic origin, due to impaired vascularization, post-traumatic, or idiopathic.
Other causes include: NSAIDs, Gaucher disease, osteomyelitis, sickle cell disease, alcoholism, coagulopathy, vasculitis, decompression sickness, Fabry disease, radiation therapy, corticosteroid therapy, Cushing syndrome, neuropathic arthropathy, gout, Langerhans cell histiocytosis, pancreatitis, hemophilia, infection.

MRI

More sensitive than scintigraphy.
Absence of post-gadolinium enhancement.
Subchondral lesion in T1 hyposignal bordered by a rim of T1 and T2 hyposignal.
Occasionally, an inner T2-hyperintense and outer T2-hypointense rim: double-line sign.
