Definition
The os trigonum is an accessory ossicle resulting from failure of fusion or incomplete fusion (synchondrosis) with the lateral process of the talus.
The secondary ossification center appears between 7 and 13 years of age and fuses with the talus within the following year.
Clinical features
Pain of the posterior aspect of the ankle may result from compression of adjacent structures during repeated plantar flexion maneuvers.
Management
Surgical removal in case of impingement
Differential diagnosis
Fracture of the posterior tubercle of the talus
