Definition
Familial symmetric lipomatosis: significant and symmetric accumulation of non-encapsulated adipose masses, predominantly cervicofacial
Rare disease, estimated incidence of 1/25,000 cases
Men between 30 and 60 years, predisposing factor: alcohol use
Pathology: similar to normal adipose tissue, thicker fibrous septa, exceptional degeneration
Frequent association with alcoholism, liver disease, glucose intolerance, hyperuricemia, upper aerodigestive tract cancer
Clinical features
Lipomatous masses: no capsule, infiltrative character, painless unless nerve or mediastinal compression
CT
Cervicofacial lipomatous infiltration
Management
The only effective treatment is surgical:
-surgical excision and liposuction,
-hemorrhagic and infiltrative nature of lipomatous masses
-alcohol withdrawal, tendency to recur, especially after liposuction
Without treatment, no spontaneous regression
