Ultrasound

Different possible presentations on ultrasound:
- Single or multiple lesion
- Renal involvement
- Diffuse infiltration
- Perirenal lymphoma
Often hypoechoic or nearly anechoic
Renal vein or IVC thrombosis is rare
Lymphadenopathy and splenomegaly
Hypovascular on color Doppler (rarely hypervascular)

CT

Contrast-enhanced CT
Reference examination for staging
Homogeneous mass with mild enhancement
Retroperitoneal lymphadenopathy or involvement of other regions
Splenomegaly
Lymphomatous involvement of other organs

MRI

Mass iso- or slightly hypointense on T1,
hypointense on T2
Minimal enhancement after gadolinium injection

Differential diagnosis

Primary renal neoplasm
Renal infarctus
Pyelonephritis, focal nephritis, renal abscess
Renal parenchymal involvement secondary to glomerulonephritis...
Other causes of perirenal collection
