Definition
Ovarian cyst = growth of a follicle or corpus luteum cyst > 3 cm.
Intracystic hemorrhage: frequent complication, potentially severe, in adolescents
Clinical features
Variable, ranging from asymptomatic to hemorrhagic hypovolemic shock, with acute abdominal pain in between
Ultrasound
Complex cystic adnexal mass
- multiple "fishnet" septa
- blood-fluid level
- or even clot
- without intralesional vascularity on color Doppler and CEUS, peripheral signal and vascularity.
Associated intraperitoneal effusion
CT
Heterogeneous adnexal mass, spontaneously hyperdense.
Preserved peripheral ovarian enhancement.
Intraperitoneal effusion or even hemoperitoneum
MRI
- iso- or slightly hyperintense on T1
- T2 may be heterogeneous
- peripheral enhancement (ovarian parenchyma)

Depending on clinical severity!

- Uropathies
- Vaginal malformations
- Digestive duplications
- Cystic teratoma
- Digestive/urologic emergencies
