Definition
Relatively common benign congenital cystic malformation resulting from a remnant of the Wolffian (mesonephric) ducts, most often located along the anterolateral wall of the vagina or the cervix.
Sometimes associated with genitourinary malformations.
Clinical features
- Most often asymptomatic, an incidental finding. - If symptomatic: urinary incontinence and recurrent urinary tract infection due to ectopic insertion of the urethra into the vagina via the cyst. Genital mass. Pelvic pain.
Radiography
Cystography: extrinsic impression on the bladder base.
Ultrasound
Anechoic collection with posterior enhancement in the vagina
CT
Rounded multiloculated lesion of fluid density along the anterolateral wall of the vagina
MRI
T2-hyperintense and T1-hypointense lesion without enhancement after gadolinium injection
Located at or above the pubic symphysis
Management
Surveillance.
Surgical treatment only if symptomatic.

Nabothian cyst
Bartholin gland cyst: below the pubic symphysis
