Definition
Encephalitis secondary to anoxia or a prolonged ischemic episode
CT
Loss of gray-white matter differentiation,
white matter hypodensity,
effacement of cortical sulci
MRI
In the acute phase:
T2 FSE and T2 FLAIR:
hyperintensity of the basal ganglia and cerebral cortex,
with sparing of the white matter.
Diffusion:
hyperintensity with decreased ADC in the basal ganglia and cerebral cortex.
T1 gado:
possible enhancement of the basal ganglia and cerebral cortex.
Perfusion:
hypoperfusion of the basal ganglia and cerebral cortex.
In the chronic (sequelae) phase:
cortico-subcortical atrophy with dilation of the ventricular system,
evidence of Wallerian degeneration,
increased ADC on diffusion imaging
