Definition
Benign, well-circumscribed parotid tumor of lymphoid origin.
Preferentially affects men in their sixties.
Often multifocal lesion (20%), generally measuring 1 to 4 cm, with frequent cystic component (30%).
Clinical features
- Painless
- Fluctuant swelling
Ultrasound
- Well-defined heterogeneous lesion with hypoechoic and isoechoic areas
- Often hypervascular on Doppler
CT
- Heterogeneous cystic lesion arising from the parotid gland
- Sharp, regular margins
- No calcifications
- Moderate contrast enhancement
- The presence of a mural nodule is highly suggestive of the diagnosis
- Tends to be multifocal / bilateral
MRI
- T1: Well-defined lesion, heterogeneous hyperintensity
- T2: heterogeneous hyperintensity
- T1 after gadolinium: no enhancement of the cystic components, the solid component may enhance
- Diffusion: no abnormality on b1000
- Perfusion:
Type A curve: ascending plateau, peak > 120s.
Type B curve: peak < 120s, wash-out ≥ 30%
Nuclear medicine
Often positive on Tc99, thallium, FDG-PET

- Adenoid cystic carcinoma
- Acinic cell carcinoma
