Definition
Deforming, non-erosive arthropathy, mainly involving the hands and feet
Classically described in acute rheumatic fever and systemic lupus erythematosus
Clinical features
Progressive, painless deformity of the hands and feet
- metacarpophalangeal (MCP) joints: ulnar deviation
- swan-neck finger deformities, "Z"-shaped thumb
These deformities are reducible by the examiner or the patient
Radiography
Deformities ++
No destructive arthritis
MRI
May be detected:
- synovitis predominating at the PIP and MCP joints
- bone erosions well visualized on T1
- bone marrow edema
- frequent tenosynovitis (tendon sheath thickening, effusion, and contrast enhancement)
Associated with the previously described deformities (clinical tab)
Management
Possible realignment surgery
Differential diagnosis
Inflammatory arthritis: destructive osteocartilaginous lesions and irreducible deformities
