Definition

Mixed sex cord-stromal tumor (Leydig cell and Sertoli cell)

Rare, 0.5% of ovarian tumors

Clinical features

Young woman < 30 years

Virilization syndrome:
- Clitoral hypertrophy
- Amenorrhea
- Hirsutism
- Breast atrophy
- Voice deepening

Laboratory findings

Hypertestosteronemia

Elevated androstenedione

Ultrasound

Ovarian lesion displacing follicles to the periphery

MRI

Variable architecture: solid, mixed, cystic

Solid component: intermediate T2 signal,
type 2 curve ++ (or 3)

Differential diagnosis

Granulosa cell tumor (estrogenic stimulation syndrome)
