Renal graft reimplantation, generally in the iliac fossa, most often on the right side.
The vascular pedicle is usually anastomosed to the external iliac vessels.

Clinical features

Post-transplant anuria
Pain in the iliac fossa

Ultrasound

B-mode: assessment of size, corticomedullary differentiation, and cortical regularityColor Doppler: check for smooth signal throughout the graftPulsed Doppler: - in the graft artery: PSV around 100 cm/s, not exceeding 150.- in the parenchyma: velocities below 40 cm/s, RI between 0.55 and 0.85, and acceleration time below 70 ms.Look for venous thrombosis, arterial occlusion, rejection, stenosis of the urinary anastomosis, hematoma, abscess, or lymphocele.

Management

In case of graft dysfunction: perform duplex ultrasound within 4 hours.
