Definition
Denervation in the territory of the anterior interosseous nerve, affecting:
- the flexor pollicis longus muscle
- the pronator quadratus muscle
- the flexor digitorum profundus muscle
Etiologies: Most often idiopathic, but may be traumatic, related to extrinsic nerve compression, or, more rarely, secondary to an intrinsic nerve lesion (tumoral)
Clinical features
Motor deficit of the muscles involved:
- Inability to perform the 'OK' sign with flexion of the thumb and index finger interphalangeal joints
- Impaired elbow pronation and flexion
MRI
Reference examination.
Denervation edema, hyperintense on STIR:
- flexor pollicis longus muscle
- pronator quadratus muscle
- flexor digitorum profundus muscle
In the long term, amyotrophy and fatty involution, assessed on T1-weighted imaging.
Search for an underlying etiology
Differential diagnosis
Other nerve entrapment syndromes:
pronator teres syndrome, carpal tunnel syndrome
