Definition
Multisystemic febrile vasculitis of unknown etiology.
Population: children +++ (50% under 2 years of age and 80% under 4 years of age)
Prognosis: cardiovascular (risk of coronary aneurysm)
Clinical features
Mucocutaneous lymph node syndrome:
- fever
- conjunctivitis
- polymorphic exanthema
- oropharyngeal enanthema
- extremity lesions
- cervical lymphadenopathy
Cardiac involvement in 25% of cases ++: pericarditis, transient myocardiopathy with heart failure and arrhythmia, coronary or other arterial aneurysms sometimes occurring early.
Ultrasound
Abdominal Ultrasound: hydrops of the gallbladder supporting the diagnosis
TTE (1/week for 4 to 6 weeks): screening for coronary aneurysms
Management
Medical treatment: aspirin / IV immunoglobulins
