Definition

Micronodular infiltration of fungal abscesses (5mm)

Clinical features

Almost exclusively in immunocompromised patients.

  • Fever.
  • Hepatomegaly.
  • Splenomegaly.
  • RUQ pain.
  • Nausea, vomiting
  • LUQ pain.
  • Jaundice.
  • May be asymptomatic.

Ultrasound

Micronodular infiltration by fungal abscesses (5 mm): multiple hypoechoic lesions.

Management

Antifungal treatment

Classification

  • Type 1: "wheel-within-a-wheel" appearance
  • Type 2: bull's-eye appearance
  • Type 3: complete central necrosis (hypoechoic)
  • Type 4: fibrous scar that may calcify (posterior acoustic shadowing++)

Fungal abscesses, tuberculosis, splenic hydatid disease, pneumocystosis, cat-scratch disease, brucellosis...
