Definition
Biceps brachialis distal tendon rupture, which normally inserts on the radial bicipital tuberosity, may be partial or complete and associated with rupture of the lacertus fibrosus (fibrous extension covering the antebrachial fascia anterior to the pronator teres)
Clinical features
Usually following a lifting effort or forced extension: sudden pain.
Chronic Popeye sign: displacement of the biceps bulge on contraction.
Ultrasound
- Tendinous strain: interruption of tendon fiber continuity, replaced either by a heterogeneous hypoechoic area or by fibronecrotic deposits and fiber debris.
- Proximal retraction of the stump (wavy, hypertrophied appearance)
- Thickening of the lacertus fibrosus.
Numerous artifacts +++ making analysis difficult.
Dynamic testing very useful.
