Definition
Malignant tumor related to PNETs (primitive neuroectodermal tumors).
Predominantly affects women under 20 years of age.
Dissemination via cerebrospinal fluid and leptomeningeal pathways is frequent.
Associations with retinoblastomas have been reported.
Clinical features
- Rarely symptomatic.
- Headache due to compression of the aqueduct of Sylvius
- Behavioral disturbances
CT
- Large lesions with ill-defined margins.
- Rare peripheral calcifications.
MRI
- Lesion with regular margins, lobulated, poorly defined and invasive, necrotic-cystic.
- T1: variable signal, often hypointense.
- T2: iso- or hypointense solid portions.
- After gadolinium injection: heterogeneous and moderate enhancement
- Diffusion: hyperintense solid portion
- T2*: possible calcifications
