Definition

Uni- or bilateral triangular iliac osteosclerosis with mild involvement of the sacral aspect, occurring mainly in women after pregnancy

Clinical features

Often asymptomatic
Possible mechanical sacroiliac pain

Radiography

Triangular osteosclerosis of the middle and lower thirds of the sacroiliac joints
Preserved joint space
Relative sparing of the sacral aspect

CT

Incidental finding Homogeneous iliac sclerosis with a regular, well-defined outer margin, often > 5 mm Intact joint space Sacral sclerosis may be present but less pronounced than on the iliac side No osteophytes

MRI

T1 and T2 hypointensity of the iliac and sometimes sacral margins

Differential diagnosis

- Inflammatory (rheumatic) sacroiliitis
