Definition

Avascular necrosis of the scaphoid, occurring in the absence of fracture or Non union

Clinical features

Pain on the radial aspect of the wrist, accompanied by stiffness and/or loss of strength

Radiography

Stage I: normal radiographic appearance

Stage II: alterations in density within the scaphoid, predominating at the proximal pole, near the radius; the shape of the scaphoid is preserved.

Stage III: loss of scaphoid shape, sometimes with fragmentation; the proximal portion of the scaphoid is collapsed with an eggshell appearance, indicative of a fracture

Stage IV: marked scaphoid collapse, with signs of radiocarpal Osteoarthritis predominating adjacent to the scaphoid

CT

CT arthrography useful for detecting chondropathy prior to the development of Osteoarthritis

MRI

Defines the extent of necrosis, which is predictive of potential revascularization

Classification

Radiographic (see above)

Differential diagnosis

Non union ++
