Definition

Avascular osteonecrosis of a vertebral body

Radiography

Uniform collapse of a vertebral body
Increased vertebral density
Sparing of the neural arch
Normal adjacent intervertebral discs above and below
Intravertebral gas vacuum, pathognomonic

CT

Indicated in cases of posterior wall retropulsion, but MRI is preferred in the presence of neurological signs

MRI

Area of osteonecrosis with T1 hypointensity, T2 hyperintensity, rimmed by a T1 and T2 hypointense band, with peripheral marrow edema showing T1 hypointensity and T2 hyperintensity.
In the presence of an intravertebral gas vacuum, it is hypointense regardless of sequence.

Differential diagnosis

Eosinophilic granuloma
Metastasis
