Clinical features

Pain with an inflammatory pattern,
swelling,
pathologic fracture,
possible medullary or radicular signs in cases of epidural or foraminal involvement

Radiography

Most often, permeative osteolysis, less commonly geographic.
Standard radiographs are sometimes normal.
Osteosclerosis and periosteal reaction are rare.
Beware of osseous localizations of Hodgkin's disease, which can be sclerotic (ivory vertebra)

CT

Less sensitive than MRI and PET-CT.
Lesion originating in the bone marrow with frequent epidural extension and extension into the surrounding soft tissues

MRI

Low T1 signal,
high T2 and STIR signal.
Areas of low T2 signal correspond to tumoral fibrosis.
Contrast enhancement after IV gadolinium injection.
Assessment of extension into the surrounding soft tissues

Nuclear medicine

PET-CT: Highly sensitive and performs staging
