Definition

  • Dilation of the saccule of the laryngeal ventricle of Morgagni
  • Supraglottic cyst lined by ciliated pseudostratified columnar epithelium
  • Internal laryngocele: intralaryngeal, bounded medially by the ventricular band and laterally by the thyroid cartilage wing
  • Mixed laryngocele: extends through the thyrohyoid membrane (between the hyoid bone and the superior border of the thyroid cartilage)
  • Air-filled cavity communicating with the laryngeal lumen, filling with secretions (laryngomucocele) that may become superinfected

Clinical features

  • Asymptomatic
  • Dysphonia
  • Lateral cervical swelling
  • Airway obstruction, particularly in cases of superinfection (laryngopyocele)

CT

Additional air-filled or fluid-filled image communicating with the larynx, arising from the laryngeal ventricle, of variable size

  • crossing the thyrohyoid membrane (= mixed laryngocele)
  • increase in size during phonation maneuvers

Management

Needle aspiration in case of inflammatory flare-up: risk of airway obstruction

Surgical resection at rest (outside the acute phase)

Differential diagnosis

Mixed laryngocele

  • Lateralized branchial cleft cyst or thyroglossal duct cyst
  • Cervical abscess (pyogenic, tuberculous)
  • Adenitis

Internal laryngocele

  • Malignant laryngeal tumor (plasmacytoma, lymphoma...)
