Definition
Cyst resulting from incomplete closure of the processus vaginalis
Benign condition, with no risk of progression to an infectious or neoplastic complication.
The processus vaginalis closes at both ends but remains patent in its middle portion, giving rise to a funicular sac with accumulation of peritoneal fluid within it (= cyst of the cord)
Always large
Clinical features
- Swelling along the spermatic cord, above the testis
- Cosmetic concern
- Mobile with the cord
- Painless, irreducible
- Often transilluminates
Ultrasound
- Not systematic
- Performed in cases of diagnostic uncertainty (particularly with an inguinal hernia)
- Anechoic lesion with posterior enhancement, thin-walled, intracanalicular, which may displace the testis within the scrotum
Management
Surgical management, not urgent
Watchful waiting in children under 2 years of age
Differential diagnosis
- Epididymal cyst
- Hydatid cyst of the spermatic cord
- Incarcerated hernia
