Definition

Syn. Enostosis, bone island, termed "giant" if larger than 2 cm. Highly prevalent pseudotumor: mainly in adults, rarely in children

Clinical features

Asymptomatic, incidental finding

Radiography

Involves all bones except the skull.
Medullary osteosclerotic lesion that may involve the cortex, round or oval, sometimes with spiculated margins, stable over time or with slow progressive growth.
No criteria of aggressiveness (lysis, periosteal reaction, soft tissue mass, etc.)

MRI

No contrast enhancement

Nuclear medicine

Most often no uptake.

Management

Possible follow-up

Differential diagnosis

Osteoblastic metastasis.
Osteopoikilosis: multiple bone islands (epiphysis and metaphysis) sparing the skull and vertebrae.
