Definition
Low bloog glucose Hypoglycemia encephalopathy.
Mismatch between glucose supply and the brain's needs, resulting in brain injury.
Clinical features
Diabetic patient ++ or glucose metabolism dysfunction.
Impaired consciousness up to coma, may be preceded by seizures.
Neurologic deficit (any type may be seen): stroke-like
Course: Prognosis varies with the duration and severity of Low bloog glucose Hypoglycemia. Prognosis correlates with involvement of the basal ganglia.
Laboratory findings
Low bloog glucose Hypoglycemia
CT
Hypodensity of the parietal and occipital lobes
MRI
T1:
- early: cortical edema, sulcal effacement,
- subacute and chronic: curvilinear cortical hyperintensity: cortical necrosis.
T2:
- hyperintensity of the parieto-occipital cortex and basal ganglia
- spares the thalami, the subcortical and deep white matter, and the cerebellum.
Diffusion: restricted diffusion with decreased ADC
Spectroscopy: decreased NAA, lactate peak
Management
Glucose correction
Differential diagnosis
Ischemic stroke
Anoxic-ischemic encephalopathy
PRES: posterior reversible encephalopathy syndrome
