Definition
Subdural CSF collection.
Causes:
traumatic, iatrogenic, arachnoid cyst rupture, intracranial hypotension.
Onset is delayed by a few days in cases of trauma.
Location: bilateral frontal +++, may be frontoparietal.
Clinical features
Often asymptomatic.
Intracranial hypertension, epilepsy, and motor deficit are possible.
CT
Subdural collection with density identical to or slightly lower than CSF.
Cerebral veins in contact with the cortex.
MRI
T2*: possible hemorrhagic changes.
T2: signal identical to CSF (hyperintense).
Cerebral veins in contact with the cortex
Management
Neurosurgical treatment in cases of mass effect or complicated arachnoid cyst.
Differential diagnosis
Chronic subdural hematoma (typically unilateral, with blood content and mass effect)
Cerebral atrophy (here the veins "cross" the cerebral peri-fluid space
