Definition
Herniation of abdominal structures through a diaphragmatic dehiscence.
1) Anteriorly located hernias:
- Morgagni hernia: failure of fusion of the medial fibers
- Bochdalek hernia: defect of the most medial fibers (course of the internal mammary pedicles)
Contents:
omentum and transverse colon
less commonly: liver, stomach, small bowel
Diagnosis:
Suprasternal abnormalities
location of digestive structures: anterior to the heart
workup: radiograph, contrast enema, and CT.
Differential diagnosis: hernia due to traumatic rupture
2) Posteriorly located hernias
hernia through the foramen of Bochdalek
symptomatic in children
asymptomatic in adults
Nontraumatic posteromedial defect of the hemidiaphragm
