Definition

Viral: HAV, B, C, D, E, but also CMV, EBV, HIV.
Drug-induced: acetaminophen++.
Toxic: alcohol

Clinical features

Incubation period: asymptomatic, normal laboratory workup.
Pre-icteric phase: flu-like syndrome.
Icteric phase with gradual onset: jaundice, dark urine, pale stools, +/- hepatomegaly, splenomegaly, lymphadenopathy

Laboratory findings

CBC: leukoneutropenia.
Liver function tests: marked cytolysis+++ > 10N predominating on ALT, cholestasis.
Etiologic workup: viral serologies

Ultrasound

hepatomegaly,
hypoechogenicity of the liver predominating in the periportal region,
gallbladder wall thickening (> 1 cm) particularly in the pre-icteric phase,
rule out CBD dilation++

CT

Periportal hypodensity.
Marked gallbladder wall thickening with mucosal enhancement and marked circumferential submucosal hypodensity
+- hilar lymphadenopathy
