Definition
Forefoot deformity combining lateral deviation of the hallux, most often associated with varus of the first metatarsal (metatarsus varus of M1).
Radiography
Weight-bearing AP radiograph of the forefoot:
- Hallux valgus = M1-P1 angle of the 1st ray >12° in men and >15° in women (physiological valgus of 8-12° in men and 8-15° in women)
- Metatarsus varus of M1 = M1-M2 angle of the 1st ray >10° (physiological varus of M1 = 5-10°)
- Splayed forefoot = M1-M5 forefoot spread angle >25° (Normal = 15-25°)
- Lateral subluxation of the base of P1 of the hallux
- Lateral subluxation of the hallux sesamoids
- Secondary osteoarthritis of the 1st MTP
- Thickening of the medial subcutaneous soft tissues overlying the 1st MTP
- Deviation and/or clawing of the 2nd toe

Weight-bearing lateral radiograph of the foot:
- Dorsal deviation of the first metatarsal on the lateral view
- Measurement of the Djian-Annonier medial arch apex angle
Normal: 120-130° - Cavus foot < 120°
- Flatfoot > 130°
