Definition
Textiloma or gossypiboma: A very rare but well-recognized postoperative complication. The reported incidence in the literature ranges from 1/1,000 to 1/10,000.
Gossypiboma is a term derived from gossypium, meaning cotton in Latin, and boma, meaning hiding place in Swahili.
It is used to describe a foreign body composed of surgical sponge(s) or drape(s) left within an operative site.
It is an iatrogenic lesion that develops around a retained textile foreign body, reflecting the inflammatory reaction induced by the textile material in contact with tissue, leading to the formation of an inflammatory granuloma.
Clinical features
Textiloma: 2 types of reactions: -Acute, florid presentation: severe abdominal pain, inflammatory syndrome. -May remain asymptomatic for a long time. Chronic obstructive or painful presentation
Ultrasound
Textiloma: well-defined hypoechoic mass containing serpiginous hyperechoic internal structures with posterior acoustic shadowing.
CT
Textiloma: variable appearances:
- Fluid collection bounded by a capsule
- Fluid collection with an air bubble
- Lesion with a serpiginous structure
- Simple cystic-appearing lesion with a slightly irregular wall raising the question of an abscess
- Internal ribbon-like structures of metallic density +++
