Definition
Congenital muscular pseudotumor: incidence 0.3% to 2%
- Firm, fibrous, non-inflammatory lesion discovered a few weeks after birth in an infant with a history of birth trauma.
- Etiology: uncertain; the most widely accepted theory is traumatic tearing during delivery
- Rarely bilateral; spontaneously resolves favorably within a few weeks to months in the vast majority of cases. Right side > left
- A possible association with developmental hip dysplasia or foot malposition suggests an asymmetric postural syndrome
IMAGING NOT ESSENTIAL FOR DIAGNOSIS
In the absence of a muscular abnormality, obtain a cervical spine radiograph (to look for a vertebral anomaly)
Clinical features
Lateral neck mass, predominantly right-sided, with torticollis and head deviation to the opposite side
Ultrasound
Fusiform thickening of the sternocleidomastoid muscle, hyperechoic compared with the contralateral muscle, with preserved fibrillar pattern
Management
No medical or surgical treatment other than postural advice
Progressive regression of the mass over approximately 4 months
