Definition
Ectopic pancreas within the duodenal wall.
Flares of pancreatitis localized to the D2 wall.
Repeated inflammation --> fibrosis.
Cause of chronic pancreatitis.
Clinical features
- Population: men in their 5th decade.
- Epigastric pain.
- Excessive alcohol consumption frequently found.
- Clinical signs: flares of acute pancreatitis (paraduodenal pancreatitis), vomiting, weight loss, jaundice
Laboratory findings
Mild abnormalities. (inflammatory markers, lipase, liver function tests).
Lipase may be normal.
Radiography
Not contributory.
CT
Abnormality localized to the pancreaticoduodenal groove:
- Infiltration of the periduodenal fat.
- Calcifications within the duodenopancreatic groove.
- Microcysts of the duodenal wall.
- Compatible clinical presentation;
Indirect signs:
- possible associated chronic pancreatitis findings (calcifications, dilation of the main pancreatic duct, pseudocysts)
- complications of chronic inflammatory lesions (gastric stasis from duodenal stenosis, biliary duct dilation from distal common bile duct stenosis).
MRI
Better visualization of the duodenal wall microcysts.
Infiltration of the duodenopancreatic groove.
Differential diagnosis
Groove pancreatitis and paraduodenal pancreatitis are synonyms for GP.
