Definition
Subcortical edema at a tendon enthesis, often related to overuse from intensive sports activity.
Location: can affect any muscle, but the classic site is the lateral femoral condyle beneath the insertion of the lateral gastrocnemius muscle
Clinical features
Asymptomatic
Laboratory findings
Normal
Radiography
Small oval cortical osteolysis surrounded by a heterogeneous subcortical sclerotic rim beneath the tendon enthesis
MRI
Inferomedial femoral metaphyseal irregularity
- Iso- to hypointense on T1
- Hyperintense on T2
- +/- sclerotic rim on both sequences
- Contrast enhancement after injection.
Bone marrow edema and intra- or peritendinous edema of the medial gastrocnemius or adductor magnus may be associated.
