Definition

Infiltration of the ACL by a mucoid matrix, with separation of its fibers, which retain a normal orientation.

Clinical features

  • Posterior knee pain
  • Discomfort on flexion
  • Sometimes asymptomatic

MRI

  • T2-hyperintense, thickened ACL.
  • Its fibers, hypointense, retain their normal orientation and remain taut.
  • "Celery stalk" appearance of the ACL.
  • Absence of indirect signs of ACL tear (bone contusions, subchondral impaction fracture of the lateral condyle, meniscal tear, anterior tibial translation, Segond fracture, ligamentous injuries...)

Differential diagnosis

Ganglion cyst of the ACL

ACL tear
