Definition
Micronodular infiltration of fungal abscesses (5mm)
Clinical features
Almost exclusively in immunocompromised patients.
- Fever.
- Hepatomegaly.
- Splenomegaly.
- RUQ pain.
- Nausea, vomiting
- LUQ pain.
- Jaundice.
- May be asymptomatic.

Cholestasis
Ultrasound
Micronodular infiltration by fungal abscesses (5 mm): multiple hypoechoic lesions.

Management
Antifungal treatment
Classification
- Type 1: "wheel-within-a-wheel" appearance
- Type 2: bull's-eye appearance
- Type 3: complete central necrosis (hypoechoic)
- Type 4: fibrous scar that may calcify (posterior acoustic shadowing++)
Differential diagnosis
Fungal abscesses, tuberculosis, splenic hydatid disease, pneumocystosis, cat-scratch disease, brucellosis...
