Definition
Pulmonary disease related to an inflammatory reaction of the distal airspaces to an insult.
Belongs to the family of diffuse interstitial lung disease (ILD)
Clinical features
- 50-60 years
- infectious presentation: fever, dyspnea, cough
- No underlying pulmonary disease
- PFTs: restrictive pattern
Laboratory findings
Bronchoscopy with BAL:
- no bacterial, viral, or fungal infection
- hyperlymphocytosis, hypercellularity,
CT
- Areas of parenchymal consolidation with air bronchogram
- More common in the lower +/- middle lobes
- Often subpleural and peribronchovascular
- Migratory
- Atoll sign and reversed halo sign
- Large nodular opacities (peribronchovascular consolidations)
- Parenchymal bands
Management
Radiological resolution under corticosteroid treatment.
Possible irreversible fibrotic lesions
Differential diagnosis
- Lymphoma
- Bronchioloalveolar carcinoma
- Eosinophilic pneumonia
- Sarcoidosis
- Vasculitis
