Definition
Destructive shoulder arthropathy - Milwaukee shoulder characterized by the presence of hydroxyapatite crystals (intra-articular calcium phosphate).
Predominant in women and elderly patients (50 to 90 years)
Clinical features
Chronic shoulder pain, worsening over a period of less than one year.
Bilateral involvement in 65% of cases.
Laboratory findings
Possible hyperparathyroidism
Radiography
Superior migration of the humeral head.
Glenohumeral joint space narrowing.
Subchondral sclerosis.
Osteophytosis (mild).
Bone fragmentation and impaction of the articular surfaces.
MRI
Subacromial-subdeltoid bursitis.
Intra-articular effusion
Management
- Joint maintenance physical therapy
- Injection during painful flare-ups
- Joint replacement
Differential diagnosis
Arthropathy secondary to a rotator cuff tear.
