Definition

Absent or incomplete pneumatization of the sphenoid sinus occurring during childhood or adolescence.
Other sinuses may be affected.

CT

Normal variant giving a pseudotumoral appearance of the sphenoid.
Easily recognized by:
- Its location: within a normally pneumatized area
- The non-expansile nature of this pseudolesion of bone
- Its peripheral sclerosis
- The absence of any compression/mass effect on adjacent anatomic structures
- The presence of a fatty component (not required for diagnosis, however); calcifications

Management

Reassure the patient from the outset
MRI / biopsy / follow-up unnecessary
