Definition
Microtraumatic or degenerative involvement of the superficial plantar fascia.
Lesions predominate in the central portion of the fascia, at its proximal enthesis on the calcaneus.
Favored by overuse in athletes, excess body weight, or hindfoot static disorders.
Clinical features
Mechanical heel pain.
Risk of partial or complete tear.
Radiography
Normal.
Occasionally calcaneal spur.
Ultrasound
Comparison with the contralateral side +++
Hypoechoic thickening of the enthesis of the superficial plantar fascia.
Occasionally Doppler hyperemia.
Anechoic areas within the fascia reflecting zones of partial tearing.
MRI
Thickening of the fascial enthesis, with +/- marked T2 hyperintensity.
Edema of the adjacent soft tissues.
